aka Posted October 4, 2013 at 03:57 PM Posted October 4, 2013 at 03:57 PM The certificate that ISP published is a PDF file that cannot be filled on the computer. Which means that you have to hand write a few hundred of these. Here is the same certificate that you can fill on your computer and print. Hopefully it will save everyone here sometime and effort. I need to make it clear that there is a possibility that ISP may object to this version of the certificate in which case you may bot use it. CCWTrainingCertificate-fillable.pdf (updated below) aka agreed to let me edit this post in order to attach the updated form below. Please use this instead of the original. AlphaKoncepts aka CGS Posted October 4, 2013 at 04:01 PM Posted October 4, 2013 at 04:01 PM Thanks very much hadn't figured out yet how to add the editable boxes and was going to simply open the PDF on Photoshop and add the information that way. Yours seems easier
Onytay Posted October 4, 2013 at 04:03 PM Posted October 4, 2013 at 04:03 PM Thank you for doing this. My handwriting skills leave a little to be desired. This should help.
AlphaKoncepts aka CGS Posted October 4, 2013 at 04:03 PM Posted October 4, 2013 at 04:03 PM There is a problem with business name, you have two text boxes on that line. Was that meant to be?
mjw45 Posted October 4, 2013 at 04:38 PM Posted October 4, 2013 at 04:38 PM This is a great help.I was thinking about seeing if we could get permission to make it a fill in word document.Tie it into a student/class database and have all the certificates filled in automatically, ready for signature. My handwriting sucks and the worst part for me in a BP class is filling out the certificates. Matt
John Q Public Posted October 4, 2013 at 05:01 PM Posted October 4, 2013 at 05:01 PM What I do is set it a word doc that does the date and most fields already and all I do is type in the student name.
CPD189 Posted October 4, 2013 at 06:02 PM Posted October 4, 2013 at 06:02 PM Thanks, this will be most helpful!
aka Posted October 4, 2013 at 07:25 PM Author Posted October 4, 2013 at 07:25 PM There is a problem with business name, you have two text boxes on that line. Was that meant to be? That is how the document was set up. I did not change anything or any of of the formatting. I will and try and fix it and post the new version.
chip Posted October 4, 2013 at 07:29 PM Posted October 4, 2013 at 07:29 PM Word on a PC or Pages on a Mac, and then setting up a Mail Merge (pulls from your student database) is absolutely the way to go. For those that are less tech savvy, this fillable PDF is a great tool. Kudos to aka for sharing.
lockman Posted October 4, 2013 at 07:29 PM Posted October 4, 2013 at 07:29 PM The ISP sent a sample format document, they did not say you had to use that one. As long as all the required information is present I do not see an issue. I would prefer the spots to check off training I did not give were not even listed on the certificate.
jazzemt Posted October 5, 2013 at 01:39 AM Posted October 5, 2013 at 01:39 AM Thank you so much for this.
Frank Posted October 5, 2013 at 02:36 AM Posted October 5, 2013 at 02:36 AM And still, no one knows what goes in the Expiration Date field! I'm tempted to make it 99 years (eg. 10/04/2112) like IDFPR does for security firearms training.
aka Posted October 18, 2013 at 03:28 PM Author Posted October 18, 2013 at 03:28 PM So ISP has released a new version of the CCW certificate this time without the "Expiration Date" that caused so much pain and suffering. And they also corrected the problem with th business name. Attached is the updated certificate that you can fill in using Acrobat Reader. Enjoy!Updated - CCWTrainingCertificate-Fillable.pdf
Tango7 Posted October 20, 2013 at 02:13 AM Posted October 20, 2013 at 02:13 AM Although most of us have completed it already, Here's a fillable version of the Livescan Authorization PDFFillableCCWInstructorFeeLiveScanAuthForm.pdf
kevinmcc Posted October 20, 2013 at 11:11 AM Posted October 20, 2013 at 11:11 AM Thanks very much hadn't figured out yet how to add the editable boxes and was going to simply open the PDF on Photoshop and add the information that way. Yours seems easier Foxit Reader is a great tool for that.
sirflyguy Posted October 21, 2013 at 10:56 PM Posted October 21, 2013 at 10:56 PM This is great. I could print out ones with everything needed and just write in a name/date and check a few boxes and sign it. Very cool.
45b Posted October 21, 2013 at 11:22 PM Posted October 21, 2013 at 11:22 PM Still a little confusing on how to fill it out. Have had other instructors ask me about the previous credit check off sections. I checked both the second and third boxes and put NRA Basic Pistol 8 hrs for those who had it, and DD214 or NGB22 8 hrs for those who had that. If I gave them both days, I just checked the 16 hr box. Any thoughts on this?
Pipedoc Posted October 22, 2013 at 01:23 AM Posted October 22, 2013 at 01:23 AM Thanks guys! The forms are much appreciated.

Any thoughts on this? I see a few ways to do this. But this is what we are doing: If you teach all 16, but the first 8 are NRA BP, I think you should check the 2nd box and indicate 8 hrs credit for NRA BP.If you teach your own approved 16 hr course, Box 1 is correct.Box 3 is for active/retired/HonDischarged MIL only.
