I was down there this morning and made my application, and saw the signs that the office is relocating to the City of Chicago Dept of Revenue neighborhood office at 4770 S Kedzie. In my case that would have been less than 5 minutes from my front door at home - timing is everything - lol For reference the office sits in the middle of strip mall, west side of the street, kitty corner across the parking lot from KFC at 47th Pl and south Kedzie. The new location is supposed to be up and running starting Sept 1st, 2010? Hindsight is a nice crystal clear 20-20, and knowing firsthand the horse-poop parking situation at 35th & Michigan, I would have waited a couple weeks and made the application locally. The new office location has a substantial parking lot with EZ access. It is also just 1 block north of the Orange Line station at Kedzie for those who want to use public transportation. You have been advised - lol OH, and just a reminder of what has been already posted, they DO require you use their yellow application form, not the one you can download from the website. And thank the gods I re-read the form or I would have forgotten to bring photocopies of my FOID and Driver's License.

Since a person is required to have an IL DL, or ID to obtain a FOID, and since the photo used for the FOID is the same as the DL regardless of the photo sent in with the FOID ap, why require both, other than a little more hassle? Ahhh, the two photos on mine are different now, as my DL renewal this Aug which required a new photo (old one was 10 yrs old) followed my FOID renewal. They'll never catch me now!!

Is the form two pages on two sheets, or one sheet both sides? I'd like to just print this out if possible, but haven't heard of anyone doing this so far.

I brought a two-sided color download print I made and had to step aside and re-write the info on their form. Kinda makes you wonder why they bother to post a PDF...don't it? LOL This whole new process was thrown together with spit, baling wire and duct tape. Not much pre-thought given to the paperwork or the mechanics of it and they seem to be making it up as they go. However the only true goal is make it a hassle for which we get to pay dearly.

Must have the yellow background. I'd try printing it on yellow stock.

A small amount of citizens are going through this whole process. I'm sure they'll have plenty of forms in stock for an eternity.

Although from the Police website, one would never know this unless they opened the document, which I did today. Other than the mention of the PRCSS (Police Records Customer Service Section) and the new address on S. Kedzie, the only other change I noticed is that in addition to the office being closed on holidays, they added that it is also closed on City shut down days. I guess the PRCSS is now the fancy name for Gun desk. The rules still state that you can download and submit the form from the website. I printed mine out on yellow card stock as suggested, usng a color printer. So it has yellow, grey, and black ink on yellow. The printed yellow part is slightly darker than the paper color, and printed actual size is just smaller than an 8.5" x 11" sheet. I fully expect them to make me fill out a real form, but I will try to submit my copy anyway, just to see if the rules are being followed to the letter. I assume there may be more amendments in the future

The gun desk will accept the CFP application if printed in color on card stock. However, they will not accept gun registration forms that were downloaded. The registration forms must be the ones given out at the gun desk. They are also card stock thickness, plus two perforations. I was surprised that they would accept one, but not the other. They were happy to give an extra, in case I messed up. I was also told that when I receive my CFP, I could register my guns and that I will need to fill out section D, and even enter a date in the "Gun was possesesed in the city by me date" box. I asked "What if they were not registered before and I never brought them to the city yet?", and that "I would bring them in the future once they were registered." I was told that it needs a date and it couldn't be a date in the future. That doesn't seem right. One positive is the electronic fingerprinting, no ink required! Although the printing also printed the side of your hands, and palm prints as well. There was another person being fingerprinted before me, but things went pretty quickly. Not a bad experience, all in all. Everyone was polite and professional. I was told it would take 3 weeks to receive the permit.
