He's under 18 with no state ID and my understanding is that a legal guardian has to call ISP and provide ID in the form of a birth certificate in order to get the correct form sent. The thing is it seems that ISP doesn't answer their phone anymore! We've tried calling many times at different times of day and we just get a recording saying they're extremely busy and we're quickly disconnected. No option to wait on the line or leave a message. Is there another route to take? It seems like they simply decided that they aren't answering the phone anymore which would amount to a defacto ban on getting a FOID like this. She does not currently have a state ID. This is a better deal as you pay $10 for a 10 year ID vs $10 for a 5 year state ID. Not to mention all the benefits of having the FOID. I believe if the minor currently has a state ID you can do it over the web access portal. Otherwise you have to call the ISP number and do the application over the phone for a minor. The number is: 217-782-7980. Once you speak with an agent, they will take the necessary information and then mail you two separate items. One will be the FOID application which will have a spot for his mother (as the sponsor) to sign as well as the applicant. If the applicant cannot sign, you will need to send a birth certificate, I believe. The second form is a minor affidavit form. His mother (as the sponsor) will need to sign this and have it notarized. They will take the payment initially over the phone as well. If you do not have a state ID, you will need to include a picture to use as well. I used a passport photo for my daughter which was taken at Walgreens.Once complete, send them back to the address on the envelope and wait....that's the part I am at so I can't give you my experience on time to completion, but should be the 30 day window...should be.... I literally dropped it in the mail this morning so I'm hoping to have it in a month.Hope that helps!Edit: clarified that his mother is the sponsor and not you. So basically if he doesn't have a state ID, yes, you do have to call. Eventually you'll get on hold (not that extremely busy line where it disconnects you) and then you'll get a person. But once they answer, it's a simple process. You give them the info for the minor, and they send you a pre-filled out form. The minor and the gaurdian sign it and send it in. I did this same process a few months ago. I don't recall getting anything notorized though. It was just a form (or maybe 2) to sign and send back in. Very easy... Xwing - Since you mention you did this a few months ago, may I inquire how long it took for you to get the card(s) in hand? I'll be coming up on mid-20 days here since I mailed it in and am just wondering when I should expect it to be the full 30 days - or longer because IL- to be completed. Thank you, sir. From start to finish, was about 1.5-2 months for the minor's FOID. But that included the time it took them to send me the pre-filled out form as well as to process it once we signed and sent it back. A notary is supposed to be a registered and trusted witness that the person signing a form is actually the person named on the form. Since the person at work knew you and who you are, they didn't have to do anything except stamp and date the form. If they didn't know you, they'd have to check your ID.
