Of course I got denied but now I'm confused about the appeal process also now that my legal name has changed. So ISP wants us to find an Illinois psychiatrist and have them send off all mental health documents. What if they don't want to do it? I'm not even sure how I'd ask a psychiatrist to submit these records. Does anyone know psychiatrists that regularly do this service, I have Molina for insurance. Also what does A current (within 45 days of ISP’s receipt) mean? ISP's receipt of what? Therefore you: submit an informal appeal, with a Mental Health Certification form (1 page, call one of the half dozen psychologists that do a lot of these), etc. You will get your FOID.
