16 Hour Training Certificate Won't Accept the Curriculum Number

I'm using the ILLINOIS STATE POLICE Concealed Carry Firearm Training Certificate, ISP 2-640 (3/18) and the form will not allow the full Curriculum Number of 13A0589.  The text box will only accept six characters rather than seven comprising the complete curriculum number.  The form in question is attached.

 

Anyone have a solution?

For future concerns, they don't ask for the CCC they only ask for CCT and that is tied to the CCC number, my bet is a CCC entered when the CCT was asked for.
