I've got a little 2 shot .357 Derringer with a custom made leather case that can we worn with a belt. The case looks nothing like a holster. In fact, it looks like a large cell phone case, with a flap that snaps. You cannot see what is inside the case. Would this be considered "concealed", even though it is worn on the outside of my clothing? And no, this is not my concealed carry weapon, but I carry a revolver, so I'm limited to 5 rounds. The requirements for legal "transport' are listed in tons of other posts on this forum. I would note that there are a number of pocket sized .380 and 9mm handguns of about the same size and weight that offer a lot more capacity than two rounds from a derringer. Which is why magazine fed firearms make more sense than a derringer in that case. Of course if you have your permit then none of that really matters. It also may be hard to get a revolver in that slim PDA style holster while still keeping its slim/low profile. There's a lot of nice pocket semi autos these days that keep those slim lines. In fact, he stated that his CCW weapon is a revolver, and he just wants to carry this as a backup. His question is one of legally concealed, not one of FOID carry.
