EnCrypt Posted May 14, 2018 at 03:26 PM Share Posted May 14, 2018 at 03:26 PM Hello, My cousin had his FOID denied because when he was about 16 he had bad anxiety and checked himself into the hospital for mental help. he stayed a day or two then went home and has been fine since. He's 25 now and would like to get a FOID card and was denied even though it's been over 5 years. he neglected to do anything about it when he got the rejection until he talked to me. i know there's a process to follow to get this resolved especially since the reason for the rejection was over 5 years ago (should not not be relevant) however i'm unfamiliar with the process. can anyone point me in the right direction? I plan on calling the ISP with him Friday but i figured this is the best place to start.
Molly B. Posted May 14, 2018 at 03:58 PM Share Posted May 14, 2018 at 03:58 PM It is a relatively easy process but not a fast one. EnCrypt Posted May 14, 2018 at 05:58 PM Author Share Posted May 14, 2018 at 05:58 PM Thanks Molly, it looks like i'll need to have him fill out the first two forms, and then this one "For denial or revocation for mental health prohibitor MORE than 5 years ago:" is that correct? i assume we have to just send this in the mail rather than some sort of digital way? Yes, his forms will have to be mailed and the one for the mental evaluation will have to be mailed straight from the doctor to the ISP.

Molly B. Posted May 14, 2018 at 07:07 PM Share Posted May 14, 2018 at 07:07 PM We recommend mailing via method that provides a signed receipt on delivery. Keep a copy of all paperwork for his records.
