Yes, a minor can apply for a FOID card with a parent or legal guardian sponsor. The parent or legal guardian must be eligible for a FOID card. The parent or legal guardian must also sign an affidavit and attach it to the application

Yes, you can. Here are the steps I posted to get my 7 year old daughter her card:

Once complete, send them back to the address on the envelope and wait....that's the part I am at so I can't give you my experience on time to completion, but should be the 30 day window...should be.... I literally dropped it in the mail this morning so I'm hoping to have it in a month.

Edit: clarified that his mother is the sponsor and not you.

 

So basically if you have a license they should have that information on file and you can use that for your picture (though you'll still have to send something in I believe). Since you're a minor, you do have to call. You may need to have your parent/guardian call...not sure. When you do call, eventually you'll get on hold (not that extremely busy line where it disconnects you) and then you'll get a person. Follow the advice above and you're good to go. Complete application over phone, sign what they send, mail it back and you'll be set.
