Heaven forbid they should test changes they make... Under the picture on the next page are several white rectangular boxes. One is named forms. Click on that or one of the other boxes, if "Forms" is not the right one. Bubbacs Posted September 2, 2020 at 11:02 PM Share Posted September 2, 2020 at 11:02 PM go to the ISP web site and traverse to the firearms section.far right is a link for person to personinput your own FOID number and birth dateIt will give you a transaction number and then show the form, print as many as you like from there.But no matter how many you print and have on hand, you will still need to come back and input the buyers ID and get a transaction number for that purchase.So I'd just grab 2 each time i do a sale and then come back if you need to later Link to comment Share on other sites More sharing options...

How did you all find out about the new ISP home page? Did I miss a post? I stumbled across it a couple of days ago when I saw a link on another illinois.gov web page. Dont have an account and they wont approve since I wont give them my mobile phone. Defeats the purpose of spreading public info. Havent been able to find a FB/social media admin/contact email to contact someone to ask them to change it.
