The employee in charge of the background checks could not log into the system, the investigator learned. The problem went unresolved until discovered by another worker in March 2017 meaning that for more than a year applications got approved without the required background check. I once worked at an insurance company where we discovered that all the people in the accounting department were using a script to log on to the accounting systems using one person's username and password. It should be fairly obvious that that's a bad thing. When we found out, we deleted the script. Then for a day, no one in the accounting department could log in, because no one could remember their own passwords. So when I read "the employee ... could not log in," what I think is "the account of the person who left 3 years ago finally got locked." Link to comment Share on other sites More sharing options...

A part of me wonders if that "employee who forgot their login" and for some reason didn't attempt to reset password or get the issue resolved for over a year and other motives for not doing so.
