My retired father's permanent residence is in Illinois. He also has a residence in Florida which he's lived at from Nov thru May for the past 17 years. Does he have to put his FL address and dates down in the residency question even though it's not his permanent address?

No.
One address for a driver's license.
One address for tax filing.
One address for a FOID and CCL.
(Correspondingly he'd put his IL address as his permanent residence on any FL forms, too.)

Addresses corresponding to what's on DL/State ID seem to work. Just make sure there's no gaps in the residency dates (which should also correspond to which addresses were on file with SOS during those dates).
