Hey All,
Another question here - sorry. I have two middle names. My drivers license shows my middle names as they are JOSHUA KEKOA. The FOID application asks you to put in your middle name. Since I have both middle names on my DL I figure I should include them in my FOID application. For the middle name field I was going to put JOSHUA KEKOA. Does anyone have two names or have experience/input on the above? My DL has my address and name in all caps so I was going to apply with it as such.

I would put your middle names in as it is on your driver's license. If it's incorrect it won't let you proceed until it's correct.

Do it the same as DL. They also use the photo from the DL.

I have this friend who has two initials for his first name, he's legally JB.
Does he put down JB or J-only B-only?

Why would he change his name by adding ONLY to both letters?
This reminds me of that thread some guy had a name which had sounds and only 3 letters for the full name!
That thread went of for months as he couldn’t tell us what the name was or the exact initials were.
But I’ve now gone of track, sorry.
