FOID denied for mistaken yes answer

Applied 9 of 19 for my Foid renewal was denied because of mistaken yes answer on disqualifer question. Letter or web site of ISPFB dosen't tell me which guestion so I can fill out correct affadivit on appeal

So you applied for renewal in sept 2019?

And you got a letter telling you nothing?

Have you tried to reapply?

And normally there is a correction button there, but if it’s yes to a disqualifier it might take more to get it turned around

I suggest you send a letter to the director of the ISP and also FAX the letter to the appeal department. In the letter, provide your full legal name, date of birth, FOID application number and your contact info. Attach a copy of the denial letter and tell them you need to know the question and a copy of the correct affidavit to file with your appeal.

The forms are handy but if the applicant doesn't know which question was answered erroneously, they can't know which form to submit with their appeal. . .
